Lincoln County Youth Advocate

The Youth Advocate program was created and established by our local school districts and the Collaborative Management Program/RISE in 2020. The Youth Advocate serves Lincoln County Schools, Limon, Genoa-Hugo, and Karval) by offering support, guidance, and bridging services to help meet the youth's needs. The target population is all students at risk of expulsion, expelled, chronically absent, and/or considered truant. The Youth Advocate meets with the youth regularly, attends all meetings/staffing, and helps navigate or bridge services to the child and their family, emphasizing safety and support. An Individualized Service Support Team (ISST) plan is offered through the Game Plan to Success (GPS) or District Attendance Review Board (DARB) meeting process. The goal of an ISST meeting is to develop an action plan to address the issues and concerns of the child(ren)/youth and family. This meeting is designed to help support families during difficult times and create a plan to support the family's concerns and needs. The plan will include but is not limited to bridging families to outside support, provider recommendations, and program(s). This meeting will prioritize the student's needs and offer service to the family by utilizing the youth and family voices through the decision-making of the plan that fits their needs.

Youth Advocate Services:

  • Provides case management to increase academic engagement

  • Offers a safe and free of judgment environment

  • Consults with teachers/staff regarding students with complex needs

  • Provides referrals to outside agencies when necessary

  • Serves as an entry-point for students experiencing challenges to be connected with resources

When to make a
Referral:

A referral to the youth advocate can be made for any student experiencing challenges or displaying concerning behaviors, such as: 

  • Chronic Absenteeism

  • Truancy

  • Runaway 

  • Expulsion

  • Suspensions

  • Disciplinary Action 

  • At-Risk of being declared habitually truant
