RISE offers facilitated Individualized Service Support Team (ISST) meetings, known as the Game Plan to Success. These meetings are designed to connect local resources, minimize duplicate services, coordinate a comprehensive service plan, and create a personalized service plan based on the needs of the child, youth, and family.  Incorporating the voices of youth and families in meetings is crucial for several reasons. Firstly, it ensures that decisions are made with a comprehensive understanding of the needs and perspectives of those directly affected. Youth and families bring unique insights and lived experiences that professionals may not fully grasp, leading to more effective and relevant outcomes.

Secondly, involving youth and families fosters a sense of ownership and empowerment. When they are actively engaged in the decision-making process, they are more likely to feel valued and respected, which can enhance their commitment to the implemented solutions.

Lastly, this inclusive approach promotes transparency and trust. It demonstrates a commitment to collaborative problem-solving and acknowledges the importance of diverse viewpoints. By prioritizing youth and family voices, we create a more equitable and responsive system that better serves the community.

what to expect

RISE will facilitate these ISST meetings. Typically, a referral is made to the Program Coordinator, who will contact the family to discuss the referral, explain the process, and schedule a meeting time. The meeting is focused on the youth and their family. Please come prepared to share your needs, concerns, and wants that will benefit your child and family. These meetings may include professionals from Lincoln County Department of Human Services, Centennial Mental Health Center, Juvenile Assessment Center, local school, or other community members that fit the child's and family's needs.

our philosophy

RISE utilizes the Family Engagement Model and serves the target population directly through an Individualized Service Support Team. With case planning aimed at resolving identified concerns as effectively and efficiently as possible, the meetings are designed to value the decision-making efforts and beliefs of that family as a whole. All attendees will discuss the child and family's strengths, needs, and concerns during the meeting. The team will create a list of potential resources and ideas that will address these areas. An Action Plan will be composed to move forward with appropriate referrals.
