The Street Outreach Program (SOP) works to identify, engage, and support youth and young adults who are experiencing homelessness, housing instability, or are at risk of becoming homeless.

Our goal is to build trusting relationships with youth by meeting them where they are, understanding their unique needs, and connecting them with resources that promote safety, stability, and long-term success.

Through outreach, case management, and community partnerships, the Street Outreach Program helps youth access:

• Safe and stable housing options

• Emergency and basic needs assistance

• Mental health and behavioral health services

• Educational support and school connections

• Employment and workforce development resources

• Life skills and independent living support

• Family support and mediation services

• Health care and community resources

What Does Street Outreach Look Like?

• Supportive, youth-centered engagement

• Information about available resources

• Safety planning

• Referrals and connections to services

• Ongoing support and case management

Participation is voluntary, and services are provided in a respectful, non-judgmental manner.

Why It Matters

Many youth experiencing homelessness are not visibly homeless. They may be couch surfing, staying temporarily with friends or relatives, living in vehicles, or moving from place to place without a stable home.

The Street Outreach Program helps ensure that these young people are connected to caring adults, supportive services, and opportunities that help them move toward stability and achieve their goals.

Through outreach, case management, and community partnerships, the Street Outreach Program assists youth in accessing:

• Emergency shelter and housing resources

• Basic needs assistance, including food, hygiene items, and clothing

• Mental and behavioral health services

• Educational support and school connections

• Employment and workforce development opportunities

• Life skills and independent living support

• Family support and conflict resolution services

• Healthcare and community-based resources

Who Is Eligible?

• The Street Outreach Program serves youth and young adults who:

• Are between the ages of 12 and 24

• Are experiencing homelessness or housing instability

• Are living in places not meant for habitation, such as vehicles, abandoned buildings, campsites, or other unsafe locations

• Are temporarily staying with friends, relatives, or others because they have no stable housing of their own (couch surfing)

• Are fleeing family conflict, abuse, neglect, exploitation, or unsafe living situations

• Are at imminent risk of becoming homeless

• Services are provided regardless of race, ethnicity, gender identity, sexual orientation, religion, disability, or socioeconomic status.
