RISE is a State of Colorado Collaborative Management Program located in Lincoln County. We support youth and their families through connection to resources and early intervention. RISE advocates for collaboration among service providers, in order to tailor support that meets the unique and specific needs of each youth and their family. Our goal is to empower families in sustaining a healthy environment of support for their children, while avoiding involvement in the juvenile justice and human service systems. Our services aim to serve children and their families in a way that promotes positive outcomes and improves family quality of life.
